Job Summary

We are seeking a compassionate and experienced Director of Social Work to join our team at the White Plains Center for Nursing in White Plains, New York. As a key member of our leadership team, you will be responsible for overseeing the social services department and ensuring the highest quality of care for our 88-bed facility's short- and long-term care residents.

Responsibilities

• Oversee the social services department, including case management, discharge planning, and care planning
• Work closely with the Administrator and interdisciplinary team to develop and implement care plans that meet the unique needs of each resident
• Ensure compliance with state and federal regulations, including MOLST and Sigma Care system
• Develop and maintain relationships with community resources and external providers
• Participate in quality improvement initiatives to improve resident outcomes and satisfaction
• Collaborate with nursing staff to ensure effective care delivery and communication

Requirements

• Master's degree in Social Work (MSW) from an accredited institution
• 3+ years of experience in healthcare, with a focus on nursing home care
• Strong working knowledge of discharge planning, care planning, MOLST, and Sigma Care system
• Excellent communication, interpersonal, and organizational skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
