About the Role
As an LPN on our 12-hour shift team, you will have the opportunity to make a meaningful difference in the lives of our patients. You will work closely with our interdisciplinary team to provide direct patient care, administer medications, and assist with daily living activities. Your strong communication skills and ability to work effectively in a fast-paced environment will help you build strong relationships with our patients and their families.
Responsibilities
- Provide direct patient care, including administering medications, taking vital signs, and assisting with daily living activities
- Develop and implement personalized care plans in collaboration with our healthcare team
- Communicate effectively with our patients, families, and healthcare team members to ensure seamless care coordination
- Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
- Maintains accurate and timely documentation of patient records
