About the Role
We are seeking a dedicated and compassionate Registered Dietitian to join our interdisciplinary team at Valley Manor Rehabilitation and Healthcare Center in Coopersburg, Pennsylvania. As a Registered Dietitian, you will be responsible for developing and implementing individualized nutrition care plans for our patients, collaborating with healthcare professionals to ensure comprehensive care, and providing education and support to patients and their families.
Responsibilities
- Develop and implement individualized nutrition care plans for patients with various medical conditions and dietary needs
- Conduct dietary assessments and monitor patients' progress
- Collaborate with healthcare professionals to develop and implement comprehensive care plans
- Provide education and support to patients and their families on nutrition and hygiene practices
- Maintain accurate and detailed records of patient care
- Participate in interdisciplinary team meetings to discuss patient care and outcomes
Requirements
- Hold a Bachelor's or Master's degree in Nutrition or a related field
- Be a registered dietitian with a current registration with the Commission on Dietetic Registration (CDR)
- Possess a current Pennsylvania Dietitian Registration
- Have at least [minimum years of experience] years of experience in a healthcare setting
- Possess strong communication and interpersonal skills
- Be able to work effectively in a fast-paced environment
