As a Licensed Practical Nurse (LPN) at The Center at Blue Hills, you will play a vital role in delivering high-quality patient care to our residents. We are seeking a compassionate and skilled LPN to join our team on all shifts, including days, evenings, and nights.
Responsibilities
- Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with activities of daily living
- Monitor and report changes in patients' conditions to registered nurses and other healthcare team members
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare team members to ensure effective communication and coordination of patient care
- Maintain accurate and detailed records of patient care and treatment
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements
- Current LPN license in the state of Massachusetts
- High school diploma or equivalent required
- Previous experience in a long-term care setting preferred but not required
