Join the Synergy HomeCare of Piscataway team and be part of a dynamic organization that prioritizes patient-centered care and compassion. As a Home Health Aide (CHHA), you will play a vital role in delivering exceptional home care services to our patients in South Plainfield, New Jersey.
Responsibilities
- Provide personalized home health care services to patients, including assistance with ADLs, medication management, and other essential care activities
- Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions and coordination
- Monitor patient vital signs, report any changes or concerns to the healthcare team, and implement care plans as directed
- Maintain accurate and detailed records of patient care, including noting progress, outcomes, and any changes or challenges
- Collaborate with other healthcare professionals, including nurses, therapists, and social workers, to ensure comprehensive care
- Demonstrate a commitment to excellence in patient care, following all necessary protocols, policies, and procedures
- Stay up-to-date with industry developments and best practices to continuously improve patient outcomes and care delivery

- 1 year of experience in a healthcare setting
- High School diploma or equivalent
- Certification as a Home Health Aide (CHHA)
