Job Summary
We are seeking a compassionate and dedicated Full-Time Caregiver to join our team at SYNERGY HomeCare in Bedford, NH. As a Full-Time Caregiver, you will play a vital role in providing high-quality care and support to our clients, enhancing their quality of life, and helping them feel safe and comfortable in their own homes. Responsibilities
- Provide personalized care and support to clients, including assistance with daily living activities, medication management, and mobility and transfer assistance
- Establish and maintain positive relationships with clients, families, and healthcare professionals
- Conduct initial and ongoing assessments of clients' needs and preferences to develop individualized care plans
- Administer medications, treatments, and other therapies as instructed
- Maintain accurate and detailed records of client care and services
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
- Participate in ongoing training and education to stay current with industry best practices and industry regulations
