  • Coordinate and deliver orientation for newly hired nursing staff and assigned home health aides; document completion and communicate readiness for assignments to clinical leadership.
  • Provide education on patient assessment, OASIS documentation, individualized care planning, orders, medication safety, infection prevention, patient rights, emergency response, and timely reporting of changes in condition.
  • Conduct or coordinate initial and ongoing competency assessments within the supervisor’s qualifications and scope of practice, using direct observation, return demonstration, record review, and other appropriate methods.
  • Provide home health aide education and competency evaluation only when qualified and authorized for the specific activity; coordinate discipline-specific instruction with appropriately qualified professionals.
  • Identify competency gaps, document corrective education and reassessment, and promptly notify clinical leadership when staff should not perform a task independently until competency is demonstrated.
  • Maintain education records that identify the topic, objectives, date, instructor, participants, completion, and evaluation or competency results; coordinate tracking and filing with Human Resources and clinical leadership.
  • Evaluate training effectiveness through follow-up observation, documentation review, and relevant QAPI measures; revise education when performance gaps continue.
  • Serve as a clinical mentor and resource for staff and reinforce patient and caregiver teaching using teach-back and return demonstration as appropriate.
