About the job
We are seeking a compassionate and motivated Licensed Master Social Worker (LMSW) to join our team. This is an in-office position providing telehealth services to patients.
Responsibilities
- Conduct patient assessments and intakes
- Provide individual counseling and behavioral health support
- Develop and follow patient care plans
- Coordinate care with healthcare providers
- Maintain accurate, HIPAA-compliant documentation
- Provide case management and connect patients with appropriate resources
Qualifications
- Active New York State LMSW license
- Strong communication and interpersonal skills
- Experience with behavioral health, counseling, or case management preferred
- Comfortable providing services via telehealth
- Organized and able to manage multiple patients
