About the Role

As an LPN + RN, you will work closely with our interdisciplinary team to provide comprehensive care to our patients. Your responsibilities will include:

• Assessing, planning, implementing, and evaluating patient care in accordance with established standards and protocols
• Developing and implementing individualized patient care plans in collaboration with the healthcare team
• Providing direct patient care , including administering medications, performing treatments, and conducting patient education
• Collaborating with other healthcare professionals to ensure seamless care transitions and effective communication
• Maintaining accurate and detailed patient records , including charting and documenting patient information

Requirements

• Licensed Practical Nurse (LPN) and Registered Nurse (RN) with a current license in the state of Illinois
• 0 years of experience , with a desire to grow and develop in a dynamic healthcare environment
• Strong communication and interpersonal skills , with the ability to work effectively with patients, families, and healthcare teams
