About the Role

As a Nurse with Hope Home Health, you will be responsible for:

• Providing skilled nursing care to patients in their homes, including wound care, medication management, and education
• Collaborating with patients, families, and healthcare teams to develop and implement personalized care plans
• Conducting initial and ongoing assessments to identify patient needs and develop strategies to address them
• Maintaining accurate and detailed records of patient care and progress
• Educating patients and families on self-care and disease management techniques
• Participating in quality improvement initiatives to ensure optimal patient outcomes and satisfaction

Requirements

• Current licensure as a Registered Nurse (RN) in Ohio.
• 1 years of experience in home health care or a related field.
• Ability to recertify and maintain current certifications and competencies
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver's license and reliable transportation
