Job Summary
Homeside Rehab is seeking a compassionate and dedicated Occupational Therapy Assistant (OTA) to provide home-based therapy services in Pound Ridge, NY 10576 .
This is an excellent opportunity for a therapy professional looking for flexible scheduling, competitive compensation, and one-on-one patient care while working with a supportive clinical and administrative team.
Job Responsibilities
- Provide skilled therapy services under the supervision and direction of a licensed Occupational Therapist
- Carry out established plans of care and treatment interventions based on patient needs and goals
- Assist patients with improving mobility, strength, functional independence, activities of daily living, and overall quality of life
- Monitor patient progress and communicate changes or concerns to the supervising therapist
- Maintain accurate and timely documentation of all patient care in accordance with regulatory and company requirements
- Collaborate with supervising therapists and the clinical team to ensure coordinated and effective treatment
- Communicate professionally with office staff, patients, and caregivers regarding scheduling and patient care
Job Qualifications
- Active New York State OTA license/certification - required
- Basic computer proficiency for electronic documentation
- Strong interpersonal and communication skills
- Ability to work independently in a home-based setting while following the established plan of care
- Reliable transportation for home visits
- Home care experience preferred
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
