We are a compassionate and growing home health care agency seeking a Licensed Physical Therapist to provide quality, one-on-one therapy services to patients in their homes. This is your chance to work independently while being part of a supportive and experienced team that values excellence in care.
Location: Greater Grand Rapids area
Schedule: Per diem
Responsibilities
- Provide skilled physical therapy services to patients in their homes
- Create individualized care plans based on functional goals
- Communicate with physicians, patients, and caregivers to ensure coordinated care
- Document visits and progress using EMR systems
Requirements
- Current State PT License
- At least 1 year of clinical PT experience preferred
- Home health experience a plus (but we provide training!)
- CPR BLS
- Valid driver’s license and reliable transportation
