Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) and Registered Nurses (RN) to join our team at Harbourwood Post-Acute and Rehab in Clearwater, Florida, United States. Responsibilities
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
- Develop and implement individualized care plans in collaboration with interdisciplinary team members
- Monitor patient vital signs and report any changes or concerns to the nursing staff
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, and grooming
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
- Collaborate with other healthcare professionals to achieve patient-centered care goals

Requirements
- Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Florida
- One (1) year of experience as an LPN or RN in a post-acute care setting preferred

- Ability to work in a fast-paced environment and prioritize tasks effectively
- Compassionate and empathetic approach to patient care
