Responsibilities
- Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and vital signs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report patient progress, including any changes or concerns
- Maintain accurate and detailed records of patient care and treatment
- Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
- Participate in quality improvement initiatives and educational programs to stay current with best practices
Requirements
- Current LPN licensure in the state of Illinois
- Minimum of 0 years of experience as an LPN (recent graduates encouraged to apply)
- Ability to work in a fast-paced environment with frequent interruptions
- Excellent communication and interpersonal skills
- Ability to lift, transfer, and reposition patients as needed
