We are seeking a compassionate and motivated Social Services Assistant to join our team and play a vital role in supporting our residents and their families. If you’re passionate about making a meaningful impact in long-term care, we’d love to hear from you.
Shift
- Monday-Friday
Key Responsibilities
- Develop and implement a comprehensive social services program for residents and families
- Provide counseling and address medical-related social needs
- Coordinate and facilitate family meetings and ongoing communication between residents, families, and the care team
- Advocate for residents’ rights and serve as a liaison among all stakeholders
- Manage discharge planning and transitions of care
- Deliver exceptional service in a supportive and respectful environment

Qualifications
- Bachelor’s degree or higher (Social Work preferred)
- Prior experience providing social services in a healthcare or skilled nursing setting
