Occupational Therapist OT Home Health (PRN)

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health OT (Occupational Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in east Pasco county (Wesley Chapel, Land' O Lakes, Odessa, Dade City, Zephyrhills, and surrounding areas).

Locations: Home health visits in east Pasco county (Wesley Chapel, Land' O Lakes, Odessa, Dade City, Zephyrhills, and surrounding areas).

Roles may also be available in West Pasco, Hernando, Hillsborough, and Pinellas counties as well!

Schedule : PRN; 5-10+ home health visits per week.

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
