PRN Home Health Physical Therapist

Bryan College Station Territory

At Elara Caring , we’re looking for a Physical Therapist to serve La Grange . Although this is a PRN position, we’re looking for a commitment to consistent availability, including the days and hours you can work each week.

Your Impact

You’ll help patients improve strength, mobility, balance, and independence by:

• Evaluating functional needs and creating individualized treatment plans
• Providing skilled physical therapy in the home
• Tracking progress and adjusting care as needs change
• Teaching patients and caregivers strategies to support recovery between visits
• Coordinating care with physicians and other healthcare professionals
• Completing timely, accurate clinical documentation
• Supervising therapy assistants and support team members as appropriate

What You’ll Need

• Graduation from an APTA-approved physical therapy program
• A current, unrestricted Texas PT license
• At least one year of experience in a healthcare facility
• A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations
• Ability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning, transfers, and moving equipment
