Job description

Concordia at Villa St. Joseph is a senior care community that provides skilled nursing and long-term care services to individuals with varying medical, rehabilitative, and personal care needs. Our team is committed to delivering compassionate, person-centered care that promotes resident dignity, safety, and quality of life. We value teamwork and collaboration across all departments to ensure residents and their families receive the support and services they need. We are in search of an experienced Social Worker to join our team.

The Social Worker is a vital member of the skilled nursing facility (SNF) team at Concordia at Villa St. Joseph. This position plays an essential role in supporting residents and families, coordinating care, addressing concerns, and promoting positive resident outcomes.

The Social Worker works closely with all departments and the interdisciplinary team (IDT) to facilitate effective communication, discharge planning, and problem-solving for both short-term rehabilitation and long-term care residents. This position advocates for resident rights, dignity, and quality of life while helping ensure safe transitions of care and individualized support throughout each resident's stay.

Essential Duties and Responsibilities of a Social Worker:

Interdisciplinary Collaboration: Work closely with Nursing, Therapy, Admissions, MDS, Case Management, Activities, and other departments to coordinate resident care, address concerns, and resolve problems.

Discharge Planning: Collaborate with the interdisciplinary team to develop and coordinate safe, appropriate discharge plans for short-term rehabilitation residents, identifying barriers and arranging necessary post-discharge services and resources.

Resident and Family Support: Maintain regular communication with residents and families, address concerns, assist with adjustment to the facility, and provide support during changes in condition or care needs.

Long-Term Care Services: Assess and address psychosocial needs, promote resident dignity and quality of life, and assist residents and families with long-term care adjustment and ongoing concerns.

Assessments and Care Planning: Complete required psychosocial assessments and participate in care plan meetings

Problem-Solving: Proactively identify concerns, collaborate with department leaders to develop solutions, and follow up to ensure issues are appropriately resolved.

Documentation and Compliance: Maintain accurate and timely documentation, complete required social services records, protect resident confidentiality, and comply with applicable regulations and facility policies.

Qualifications of a Social Worker:

Bachelor's degree in Social Work or a related field, as required by applicable regulations.

Previous experience in skilled nursing, long-term care, or rehabilitation preferred.

Strong communication, organizational, interpersonal, and problem-solving skills.

Ability to collaborate effectively with residents, families, and an interdisciplinary team.

Ability to manage multiple priorities while maintaining confidentiality and completing required documentation.

Expectations for Success for the Social Worker:

The Social Worker is expected to be a proactive and dependable member of the facility team. Success requires strong communication, consistent follow-through, and active collaboration with all departments to address resident and family concerns, overcome discharge barriers, and promote positive outcomes for both short-term rehabilitation and long-term care residents.

The overall goal is to support resident well-being, strengthen interdisciplinary teamwork, and ensure safe, effective, and person-centered care at Concordia at Villa St. Joseph.
