Community Skilled Health Care Centre is now hiring Licensed Practical Nurses (LPNs) to join our dedicated nursing team. As an LPN in our skilled nursing facility, you will play a key role in delivering high-quality resident care and supporting a strong healthcare team in a welcoming, well-managed environment.
Responsibilities
- Provide direct nursing care to residents in a long-term care and skilled nursing setting
- Administer medications and treatments as prescribed by physicians
- Monitor residents’ conditions, recognize changes, and report to the nursing supervisor or RN
- Collaborate with RNs, STNAs, and other healthcare professionals to ensure quality, patient-centered care
- Maintain accurate, detailed medical records and documentation in compliance with regulations
- Support rehabilitation and restorative nursing care programs
- Participate in care planning and assist with care coordination
Qualifications
- Active Licensed Practical Nurse (LPN) license in Ohio
- Strong clinical, communication, and critical thinking skills
- Ability to work effectively in a team nursing environment
- Previous long-term care or rehabilitation nursing experience preferred, but not required
