RN Case Manager ($10,000 Sign On Bonus)

Community Care is hiring a RN Case Manager to join our Family Care Program team. This is a full-time opportunity working out of our Dane County location. $10,000 Sign On Bonus

RN Case Manager Job Responsibilities:

Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.

Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.

RN Case Manager Job Requirements:

Education: Associates degree in Nursing Required, Bachelors Degree preferred.

Licensure: Wisconsin Licensed Registered Nurse

Licensure: Eligibility for long-term care functional screener certification is required

Licensure: WI Drivers License

Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity RequiredPreferred 2 years of work in community nursing

RN Case Manager Job Schedule:

Monday- Friday 8:00 AM-4:30 PM.
