CareCore at Willowood is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our exceptional in-house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact in the lives of residents in a skilled nursing and long-term care setting.
Position Highlights
- PRN (as-needed) schedule
- Flexible weekday and weekend opportunities
- In-house rehabilitation program
- Skilled Nursing & Long-Term Care setting
- Collaborative interdisciplinary team
- Opportunity to pick up additional hours as available
Responsibilities
- Perform comprehensive physical therapy evaluations
- Develop and implement individualized treatment plans
- Provide evidence-based therapy services to help residents improve mobility, strength, balance, and functional independence
- Collaborate with physicians, nursing staff, therapy team members, residents, and families
- Complete accurate and timely documentation using Net Health
- Utilize PointClickCare (PCC) as part of the interdisciplinary care process
- Maintain compliance with all federal, state, and company regulations
Qualifications
- Current Ohio Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program
- Skilled nursing or long-term care experience preferred, but not required
- Excellent communication, documentation, and clinical decision-making skills
- Experience with Net Health and PointClickCare (PCC) is preferred
