Join Our Rehabilitation Team!
Care Connection Rehabilitation Center is seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary rehabilitation team. This PRN opportunity is ideal for a PT who wants to make a meaningful impact while providing high-quality care in a supportive skilled nursing environment.
Responsibilities
- Evaluate patients and assess physical conditions, mobility, and functional limitations
- Develop and implement individualized treatment plans
- Provide therapeutic exercises, stretching, and hands-on treatment
- Monitor patient progress and modify treatment plans as needed
- Document evaluations, treatments, and patient progress accurately
- Educate patients and families on therapy goals, home exercises, and recovery
- Collaborate with nursing and other members of the interdisciplinary care team
Qualifications
- Doctor of Physical Therapy (DPT) from an accredited program
- Current New Jersey Physical Therapist license
- Minimum 2 years of experience preferred
- Skilled nursing or nursing home experience preferred
- Strong clinical assessment, communication, and interpersonal skills
- Familiarity with SigmaCare and Net Health preferred
