About the Role
We are seeking a compassionate and skilled Home Health Caregiver to join our team of caregivers. As a Home Health Caregiver, you will play a vital role in providing one-on-one care to our clients in the comfort of their own homes. Your responsibilities will include:
- Providing assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
- Assisting with medication management
- Maintaining a clean and safe living environment
- Encouraging independence and supporting clients in their daily routines
- Communicating effectively with clients, families, and other healthcare professionals
- Developing and implementing personalized care plans in collaboration with our interdisciplinary team
Responsibilities
- Provide high-quality, patient-centered care to clients in their homes
- Collaborate with our interdisciplinary team to develop and implement personalized care plans
- Maintain accurate and detailed records of client care
- Participate in ongoing training and education to stay current with industry best practices
