We are seeking a compassionate and experienced Care Manager to join our team in Baton Rouge, LA. As a Care Manager, you will play a crucial role in ensuring the health and well-being of our clients by managing their care plans and providing exceptional support.
About the Role
As a Care Manager, you will be responsible for:
- Coordinating and managing client care plans, including developing and implementing personalized care plans to meet each client's unique needs
- Conducting initial and ongoing assessments to identify client needs, strengths, and goals
- Developing and maintaining relationships with clients, families, and healthcare providers to ensure seamless care coordination
- Identifying and addressing any gaps in care, ensuring continuity and quality of services
- Monitoring client progress and making adjustments to care plans as needed
Responsibilities
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Communicate effectively with clients, families, and healthcare providers to ensure a seamless care experience
- Identify and address any concerns or issues that may arise during the care process
- Maintain accurate and up-to-date records of client care and progress
- Collaborate with the management team to develop and implement new processes and protocols
- Rotating on call
- Assist with recruiting and training
