Job Summary
BrightCare HomeCare is seeking a compassionate and skilled Certified Nursing Assistant Home Health Aid to join our team of dedicated professionals in providing exceptional home care services to individuals in need. As a key member of our care team, you will be responsible for delivering high-quality care in the comfort of our clients' homes, promoting independence, dignity, and overall well-being.
About the Role
As a Certified Nursing Assistant Home Health Aid , you will
- Provide personalized care services to clients in the comfort of their own homes, including assistance with activities of daily living (ADLs), personal care, and homemaking tasks
- Monitor and report changes in clients' conditions to registered nurses and other healthcare professionals
- Collaborate with healthcare teams to develop and implement individualized care plans
- Maintain accurate and up-to-date records of client care services and progress
- Provide emotional support and companionship to clients and their families
Responsibilities
- Deliver high-quality, patient-centered care to clients
- Demonstrate strong communication and interpersonal skills
- Work effectively as part of a multidisciplinary care team
- Maintain confidentiality and uphold professional boundaries
- Participate in ongoing training and education to stay current with industry best practices
Requirements
- Current certification as a Certified Nursing Assistant (CNA) in the state of Louisiana
- High school diploma or equivalent
- At least 3 years of experience in home health care or a related field
- Ability to travel to client homes within the Baton Rouge, LA area
