Responsibilities
- Coordinate and manage the care plan for clients, ensuring their unique personal and health needs are met
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless transitions and optimal care delivery
- Develop and maintain positive relationships with clients, families, and caregivers to promote a sense of trust and respect
- Monitor and evaluate client progress, making adjustments to the care plan as necessary
- Collaborate with our multidisciplinary team, including nurses, therapists, and caregivers, to ensure comprehensive care delivery
- Maintain accurate and detailed records of client interactions, care plans, and client outcomes
- Participate in ongoing training and education to stay current with industry best practices and regulations
