Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Beavercreek Health and Rehab, a premier care community in Beavercreek, Ohio. As an integral part of our care team, you will play a vital role in providing high-quality patient care, supporting our residents' physical and emotional well-being, and promoting a positive living environment.
Responsibilities
- Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians
- Conduct routine vital sign checks and document patient information accurately
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with other healthcare professionals, including physicians, physical therapists, and occupational therapists, to ensure comprehensive care
- Communicate effectively with residents, families, and staff to ensure open and transparent care
- Maintain accurate and up-to-date medical records and reports
- Participate in quality improvement initiatives and maintain high standards of care
- Support and promote a positive and engaging environment for residents and staff
