Job Summary
At Avir at Santa Fe, we are seeking a compassionate and skilled Registered Nurse or Licensed Vocational Nurse to join our dedicated healthcare team. As a premier skilled nursing facility, we provide personalized care to our residents from short-term rehabilitation to long-term care. We are committed to delivering high-quality care and exceeding our residents' expectations.
Responsibilities
- Provide high-quality, patient-centered care to our residents, including administering medications, performing treatments, and implementing care plans
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each resident
- Monitor and report changes in residents' conditions to ensure timely interventions and optimal outcomes
- Maintain accurate and detailed records of residents' care, treatments, and medications
- Participate in quality improvement initiatives to optimize patient outcomes and improve our care delivery processes
