Case Manager (IDD Services)

Pay: $50,000 to $65,000 per year. Your salary in this range depends on your experience.

Schedule: Full-time, in our Brick office, with some field visits for Support Coordinator meetings and community events.

About the job

ASG Home Care is a DDD-approved provider in New Jersey. We support adults with intellectual and developmental disabilities at home and in the community. This is a hands-on coordination role, not back-office admin. You keep services running smoothly for the people we serve and build strong relationships with Support Coordinators.

What you will do

• Coordinate DSP staffing, schedules and coverage
• Be the main contact for families and Support Coordinators
• Handle intake for new referrals, from first call to service start
• Work with our operations and billing teams
• Track compliance, documentation and service quality
• Build relationships with Support Coordinators and SC agencies, and respond to referrals quickly
• Keep referral partners informed, including when we can't fill a request

What you need

• Bachelor's degree (required)
• 5 or more years in home care, IDD services, social services, case management or care coordination (required)
• NJ DDD experience strongly preferred
• Relationships with NJ Support Coordinators a plus
• Organized, good at follow-up, and comfortable on the phone
• Valid driver's license
