Care Coordinator DDD Services

Pay: $18 to $23 per hour. Your rate in this range depends on your experience.

Schedule: Full-time and part-time positions available, in our office. Tell us the days and times you can work.

About the job

ASG Home Care is a DDD-approved provider in New Jersey. We support adults with intellectual and developmental disabilities at home and in the community. As a Care Coordinator, you work in our office to keep services running smoothly for the people we serve and their families.

What you will do

• Coordinate DSP staffing, schedules and coverage
• Be the main contact for families and Support Coordinators
• Handle intake for new referrals, from first call to service start
• Work with our operations and billing teams
• Track compliance, documentation and service quality
• Keep Support Coordinators informed, including when we can't fill a request

What you need

• Bachelor's degree preferred
• 2 or more years in home care, IDD services, social services, case management or care coordination (required)
• NJ DDD experience strongly preferred
• Organized, good at follow-up, and comfortable on the phone
