• Schedule: Day-Shift | Full-Time
• Execute personalized care plans by monitoring vitals, tracking progress, and administering clinical orders to ensure high-quality patient outcomes.
• Coordinate multidisciplinary care teams across specialties to ensure seamless communication, prompt follow-through, and holistic support for every patient.
• Champion discharge and transition planning alongside physicians, families, and case managers to set patients up for safe, sustained recovery at home.
• Communicate condition changes proactively to physicians and nursing staff to ensure rapid clinical interventions and continuity of care.

Licensure / Certification / Registration:

Required Credential(s)

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
