Job Description
The Licensed Practical Nurse (LPN) provides direct and indirect skilled nursing care to residents in a long-term care setting. The LPN must demonstrate substantial knowledge of the principles of growth and development throughout the life span and the ability to assess and interpret patient information reflecting their age-specific needs. The position entails maintaining collaborative relationships with nursing staff, doctors, and other ancillary staff while following defined policies and procedures under the supervision of a Registered Nurse.
Responsibilities
- Collect ongoing data relevant to residents' physical and psychosocial conditions, communicating findings to the Registered Nurse and interdisciplinary team.
- Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and interventions, involving residents and their families in the process.
- Recommend revisions to care plans based on evaluations of resident outcomes, ensuring comprehensive care.
- Maintain competency in nursing practice, potentially mentoring or orienting new staff members.
- Provide leadership in a professional practice setting under the direction of the Registered Nurse.
Required Experience
- One year of experience as a Licensed Practical Nurse in a long-term care facility.
Certifications/Licensure
- Current New York Licensed Practical Nurse (LPN) licensure.
- Basic Life Support (BLS) certification through the American Heart Association (AHA) or RQI.
