Schedule/Shift
Part Time Nights: 2 shifts of 8 hours and 3 shifts of 8 hours one week; 2 shifts of 8 hours the following week. Every other weekend required (Friday and Saturday nights).
Job Description
The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care to residents in a long-term care setting. The position requires the ability to assess, interpret, and address the care needs of adult and geriatric patients while promoting a healthy and safe environment. The LPN will work collaboratively with other nursing staff, physicians, and ancillary teams to develop and implement patient care plans alongside families and healthcare staff.
Responsibilities
- Collect and communicate ongoing assessments of residents' physical and psychosocial conditions to the Registered Nurse (RN) and interdisciplinary team.
- Contribute to the creation of age-appropriate care plans reflecting nursing diagnoses and interventions to meet desired outcomes.
- Recommend revisions to care plans based on resident evaluations and identify the need for referrals.
- Maintain knowledge and competency in nursing practice, including participation in mentoring and orientation of staff under RN supervision.
Required Experience
- 1 year of experience as a Licensed Practical Nurse in a long-term care facility.
Certifications/Licensure
- Current New York LPN Licensure.
- BLS certification (AHA or RQI only accepted).
