Ameriwell Care is a growing New Jersey Division of Developmental Disabilities (NJ DDD) service provider dedicated to delivering reliable, personalized support to individuals with disabilities and their families.

We are looking for an experienced, organized, and motivated NJ DDD Case Manager & Referral Coordinator to join our team in Lakewood.

This position combines case management, staff coordination, and business development. The ideal candidate understands the NJ DDD system, enjoys building relationships, and can independently manage cases while helping our agency grow.

Key Responsibilities

Case Management & Operations

• Oversee active client cases and ensure services run smoothly.
• Maintain regular communication with participants, families, support coordinators, and DSPs.
• Coordinate staffing, schedules, and service coverage.
• Manage new client intake, onboarding, and service coordination.
• Monitor service authorizations, documentation, and compliance requirements.
• Address concerns and resolve day-to-day staffing and service issues.
• Work with HHAeXchange and other agency systems.

Referral Development & Agency Growth

• Build and maintain strong relationships with NJ DDD support coordinators and support coordination agencies.
• Follow up on new referrals and prospective clients.
• Promote Ameriwell Care's services throughout New Jersey.
• Help convert incoming referrals into successfully staffed cases.
• Identify opportunities to expand agency services and strengthen referral relationships.

Qualifications

Previous professional experience working with NJ DDD is REQUIRED.

• Strong understanding of NJ DDD services, support plans, and the provider system.
• Experience with a DDD service provider or support coordination agency strongly preferred.
• Familiarity with HHAeXchange preferred.
• Excellent communication, organizational, and problem-solving skills.
• Ability to multitask, manage cases independently, and follow through on responsibilities.
• Strong interpersonal skills and a proactive, growth-oriented mindset.
• Existing relationships with NJ DDD support coordinators are a plus.

Important: Applicants must have direct NJ DDD experience. General healthcare or home care experience alone does not meet this requirement.
