We are seeking a licensed Physical Therapist (PT) who is passionate about delivering high-quality, patient-centered care directly to local families and kupuna.

Responsibilities
- Delivering skilled, compassionate in-home physical therapy services.
- Conducting comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments.
- Designing and executing individualized treatment plans focused on balance, strength, and spatial safety.
- Empowering patients and their multi-generational caregivers through tailored home exercise program education.
- Ensuring accurate, compliant clinical documentation and collaborating smoothly with our interdisciplinary care team.
What We Are Looking For
- Active, Hawaii license as a Physical Therapist (PT).
- Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds).
- Excellent communication, time management, and independent clinical documentation skills.
- Valid driver’s license and reliable transportation to travel to patient homes.
- A genuine dedication to serving seniors and giving back to our local Oʻahu community.
