We are seeking a dedicated and compassionate Registered Nurses to join our team at Alden Estates of Jefferson in Jefferson, Wisconsin for PM and NOC shifts. As a Registered Nurse at Alden Estates of Jefferson, you will play a vital role in providing exceptional short-term rehabilitation, transitional care, and post-acute services to our residents.
About the Role
Responsibilities will include
- Conducting clinical assessments and care planning for residents
- Participating in medication management, admission/discharge details, and vital sign monitoring
- Providing weights, vitals, skin care, and reporting changes in resident behavior or status
- Contributing to our Quality Assurance Performance Improvement process to ensure continuous improvement
- Working closely with our multidisciplinary team to deliver patient-centered care
