Palliavte Care LPN

University Home Care Inc is seeking a dedicated and compassionate Palliative Care LPN to join our team in Macomb, MI. This role involves providing high-quality nursing care to patients with serious illnesses, focusing on improving their quality of life through symptom management and emotional support.

Responsibilities

• Provide direct nursing care and symptom management to patients with life-limiting illnesses.
• Collaborate with physicians, interdisciplinary teams, patients, and family members to develop and implement individualized care plans.
• Monitor patient health status and report changes to appropriate healthcare providers.
• Educate patients and families on disease progression, treatment options, and comfort measures.
• Maintain accurate and timely patient records in compliance with regulatory standards.
• Support patients’ emotional and psychosocial needs in a compassionate manner.
• Ensure adherence to infection control and safety procedures.

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of Michigan.
• Experience in palliative care, hospice, or related nursing field preferred.
• Strong communication and interpersonal skills.
• Be able to travel to Wayne & Oakland County
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate, patient-centered approach to care.
• Basic computer proficiency for documentation and communication.
