We are seeking experienced and compassionate Registered Nurses (RNs) to join our growing home care team as Home Care Case Managers . This is an excellent opportunity for nurses who are passionate about coordinating care, supporting patients in the community, and making a meaningful impact on patient outcomes.

Responsibilities

• Manage and coordinate patient care plans in a home care setting
• Conduct patient assessments and ongoing evaluations
• Collaborate with physicians, caregivers, and interdisciplinary teams
• Monitor patient progress and ensure quality care delivery
• Maintain accurate and timely clinical documentation
• Educate patients and families on treatment plans and health management

Qualifications

• Active Connecticut RN License
• Home Care or Hospital Case Management experience preferred
• Strong assessment, communication, and organizational skills
• Ability to work independently and manage multiple cases
• Bilingual candidates are encouraged to apply
