The Center at Blue Hills is seeking a compassionate and skilled Licensed Practical Nurse (LPN) / Registered Nurse (RN) to join our talented team of healthcare professionals. Job Summary
We are seeking a dedicated and experienced LPN/RN to work in a fast-paced and dynamic long-term care environment. As an LPN/RN at The Center at Blue Hills, you will be responsible for providing high-quality nursing care to our residents, managing medications, observing and reporting changes in resident condition, and collaborating with other healthcare professionals to ensure comprehensive care.
Responsibilities
- Provide direct patient care to residents, including assessments, medications, and treatments
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report changes in resident condition to the physician and other healthcare professionals
- Maintain accurate and up-to-date records of resident care and treatment
- Participate in quality improvement initiatives and maintain ongoing education and training
- Demonstrate a commitment to our residents' physical, emotional, and social well-being
