Job Summary
We are seeking a compassionate and dedicated Home Caregiver to join our team at SYNERGY HomeCare in Bedford, NH. As a Home Caregiver , you will have the opportunity to make a meaningful difference in the lives of our clients by providing high-quality, personalized care in their own homes. Our Home Caregivers are the heartbeat of our organization, and we are committed to supporting them every step of the way.
Responsibilities
- Provide non-medical care and assistance to clients in their homes, including personal care, light housekeeping, and meal preparation
- Assist clients with daily living activities, such as bathing, dressing, and grooming
- Monitor and record client vital signs and report any changes to the care team
- Assist clients with medication management and daily routines
- Facilitate socialization and companionship to promote emotional well-being
- Provide emotional support and companionship to clients and their families
- Collaborate with the care team to develop and implement personalized care plans
- Maintain accurate records and reports of client care and services provided
