As a Licensed Practical Nurse (LPN) at Spring Creek Healthcare Center, you will play a vital role in delivering exceptional care to our residents. Our state-of-the-art facility is committed to providing a safe, supportive, and compassionate environment for our patients, and we seek a dedicated and caring professional like you to help us achieve this goal.
Responsibilities
- Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report changes in patient condition to the Charge Nurse or Physician
- Maintain accurate and detailed records of patient care and treatment
- Participate in quality improvement initiatives and patient satisfaction surveys
- Collaborate with other healthcare professionals to ensure seamless care transitions
