About the job
We are seeking a compassionate and experienced Speech-Language Pathologist (SLP) to join our dedicated team in a long-term care/skilled nursing facility setting. The ideal candidate will be passionate about helping patients improve their quality of life and meet all certification requirements.
Responsibilities
- Evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders
- Develop and implement individualized treatment plans
- Collaborate with interdisciplinary teams to ensure high-quality patient care
- Document evaluations, treatment sessions, and progress using Net Health (Therapy EMR system)
- Participate in care planning and family conferences as needed
Requirements
- Master’s Degree in Speech-Language Pathology
- ASHA Certification (CCC-SLP) required
- VT state licensure as a Speech-Language Pathologist
- Previous experience in LTC/SNF setting preferred
- Proficiency with Net Health or similar electronic documentation systems is a plus
