Experienced Home Health Physical Therapist (PT)
One Home Health serves patients throughout Porter County and surrounding communities in Northwest Indiana . Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.

Compensation
Job Type: Part-Time, PRN/Per Diem
Pay: $60.00 - $80.00 per hour
Location: Porter County and Surrounding Northwest Indiana Communities
Compensation is based on experience, qualifications, and clinical expertise.

Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Provide skilled physical therapy interventions according to physician orders and established care plans.
- Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
- Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
- Monitor patient progress and adjust treatment plans based on clinical findings.
- Complete accurate, timely, and compliant clinical documentation.
- Participate in discharge planning and patient outcome evaluations.
Qualifications
Required
- Active Illinois Physical Therapist (PT) license.
- Graduate of an accredited Physical Therapy program.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Excellent communication, organization, and time-management skills.
- Ability to work independently in a home health setting.
Preferred
- Home health experience preferred.
- Experience with Medicare home health regulations and documentation requirements.
- Knowledge of OASIS documentation.
- Experience with Axxess software.
- Strong orthopedic, neurological, geriatric, or rehabilitation background.
- Bilingual candidates are encouraged to apply.
