Social Worker (CSW/LSW)

The CSW/LSW Social Worker provides support, counseling, and case management services to individuals, families, and groups. We are seeking a dedicated and compassionate CSW/LSW Social Worker to join our team of professionals. The ideal candidate will be passionate about working with the senior population and committed to providing high-quality support and care.

Currently Hiring for Part Time or Per Diem

Compensation

• $35 which is negotiable and can be discussed further during the interview.

Key Responsibilities

• Conducting admission and ongoing assessments, including documentation
• Assisting residents with adjustment to the facility
• Completing and maintaining MDS 3.0 documentation
• Developing and participating in care planning
• Assisting with Medicaid applications and other financial matters
• Advocating for residents’ needs and rights
• Providing crisis intervention when necessary
• Coordinating and supporting discharge planning

Qualifications

• Must hold a valid New Jersey Social Work Certification (CSW/LSW) in good standing
• Strong organizational skills with the ability to multitask effectively
• Passion for working with the senior population
• Excellent writing and documentation skills
• Experience working in a Nursing home Facility is preferred
