At North Lake Care Center , we are dedicated to setting standards in home health care, and we are seeking a compassionate and skilled Per Diem LPN to join our team. As a key member of our care center, you will play a vital role in providing high-quality care to our residents.

About the Role

• As a Per Diem LPN , you will work variable shifts, including days, evenings, and weekends, to support our hospital's needs.
• You will provide direct patient care, including administering medications, performing treatments, and providing wound care.
• You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
• You will maintain accurate and detailed records of patient care and medications administered.

Responsibilities

• Assess and document patient condition, including vital signs, and report changes to the RN or physician.
• Administer oral, topical, and injectable medications as ordered by the physician.
• Perform basic nursing skills, such as taking blood pressures, injecting medications, and applying dressings.
• Provide wound care, including changing dressings and applying topical treatments.
• Develop and implement individualized care plans in collaboration with the RN and other healthcare professionals.
• Maintain accurate and detailed records of patient care and medications administered.
