Job Summary
As a Licensed Practical Nurse (LPN), you will be responsible for delivering high-quality care to our residents within our long-term care facility. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our residents receive the best possible outcomes.
Responsibilities
- Provide direct patient care, including administering medications, performing vital signs, and conducting wound care
- Collaborate with the healthcare team to develop and implement individualized care plans
- Perform routine clinical tasks, such as taking temperatures, blood pressures, and pulse oximetry
- Document patient information accurately and thoroughly
- Communicate effectively with patients, families, and healthcare team members
- Participate in quality improvement initiatives to optimize patient outcomes
- Work flexible schedules, including days, nights, and weekends
Requirements
- Current LPN license in the State of Indiana
- Floor nurse experience preferred
- Ability to work 12-hour shifts
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Commitment to maintaining confidentiality and adhering to HIPAA regulations
