New York Community Care is seeking a compassionate and dedicated Social Worker/Case Manager to join our team as an Adult Case Manager , providing support, advocacy, and coordination of services for adult clients.
Responsibilities include
- Coordinate services and resources for adult clients
- Assess client needs and develop individualized service plans
- Advocate for clients and connect them with community resources
- Maintain accurate case records and documentation
- Collaborate with families, healthcare providers, and community organizations
- Provide culturally sensitive support to Russian-speaking clients
Qualifications
- Fluent in Russian and English
- Bachelor’s or Master’s degree in Social Work, Human Services, Psychology, or related field preferred
- Experience in case management, social services, healthcare, or adult services preferred
- Excellent communication and organizational skills
- Compassionate, dependable, and client-focused
