About the Role
As an MDS Coordinator RN, you will play a critical role in ensuring the seamless coordination of patient care by facilitating the Minimum Data Set (MDS) process. Your responsibilities will include:
- Collecting and reviewing patient information to complete the MDS assessment
- Ensuring accurate and timely completion of the MDS assessment
- Maintaining accurate and up-to-date records of patient care
- Collaborating with the interdisciplinary team to develop individualized care plans
- Providing education and support to patients and families
- Participating in quality improvement initiatives to optimize patient outcomes
Requirements
- Current RN licensure in the State of Pennsylvania
- 1+ years of experience or related experience in long-term care or rehabilitation setting
- Ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
- Strong analytical and problem-solving skills
