About the Role
As a Home Health Aide with Modani Care, you will
- Provide personalized care and support to clients in their homes, assisting with activities of daily living such as bathing, dressing, and grooming
- Assist clients with medication management, meal preparation, and light housekeeping
- Monitor clients' vital signs and report any changes to our clinical team
- Maintain accurate and confidential records of client care and services provided
- Collaborate with our experienced care team to develop and implement personalized care plans
- Demonstrate empathy, compassion, and understanding in all interactions with clients and their families
