What You'll Do

As a Homecare Registered Nurse (RN), you will

• Provide skilled nursing care to patients in their homes
• Complete comprehensive patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies as ordered by physicians
• Monitor patient conditions and communicate changes to the interdisciplinary care team
• Develop, implement, and update individualized plans of care
• Educate patients and family members on disease management, medications, and treatment plans
• Perform wound care, tracheostomy care, enteral feeding, and other skilled nursing procedures as needed
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes
• Promote patient safety, independence, and quality of life

Qualifications

• Current Pennsylvania Registered Nurse (RN) license in good standing
• Current BLS/CPR Certification
• Minimum one year of nursing experience preferred
• Previous homecare, hospice, long-term care, rehabilitation, pediatric, medical-surgical, ICU, or acute care experience is a plus
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently in a home setting
• Excellent time-management and organizational skills
• Reliable transportation and dependable attendance
